For adult learners, I often use the book "It's never too late to play Piano" by Pam Wedgwood. This comes with a helpful CD.
For younger learners, we normally start with "Tunes for 10 Fingers" by Pam Wedgwood. I find that both adults and children develop their reading much faster by also starting work on the written music theory books.
